Instructions for Completing Caption Sheet

 

 

 

 

 

The caption sheet must be word-processed and include the following: Title of Assignment, Student name, I.D. number, Standard/s, E.S.L.R. (Write out E.S.L.R.), a brief description of the assignment and a reflection on what was learned and its significance.  (See model)

 

 

 

Purpose of Assignment:

Indicate the year and class you completed the assignment or project.  Explain what the paper or project required.  (Be sure to include enough details so your readers can easily understand not only the assignment, but also the steps taken to complete it.)

 

Reflection:

Show how the assignment meets the particular E.S.L.R. and the standard being addressed. Analyze what was learned by completing this particular assignment. Show how this piece of evidence connects to future goals.  (Student will include an analysis of the skills used and further developed through the process of completing the assignment.  Student should show how those skills connect to future goals, plans or career.) Evaluate the significance of the piece submitted as evidence. (Why is this particular project or paper important enough to include in his/her portfolio?  What were the benefits of doing this particular work?)

 

Special Note:  Caption Sheets will be completed in the class the paper or project was assigned.  Teachers in all subject areas are responsible to see that students attach caption sheets to all possible Senior Exit Portfolio assignments.  This ensures that the process matches the assigned work.
